How this clinic handles your health information, and what you can ask for.

Ashray Family Clinic. The clinic decides what is recorded and why; the software is provided by a processor acting only on the clinic's instructions.
Your name, contact details, date of birth and the clinical record of your care — visits, notes, prescriptions and documents. Your health information is sensitive personal data and is treated as such.
To provide your care, to contact you about your appointments, and to keep the records the law requires a clinic to keep.
Appointment messages are part of your care. Health camps, screening drives and offers are separate, need your consent, and can be switched off at any time without affecting your appointments.
Clinical records are kept for the period Indian medical record rules require. Nothing clinical is deleted; a correction is recorded as a correction, so the history stays readable.
You may ask for a copy of what we hold, ask for a correction, withdraw consent for non-essential messages, and nominate someone to act for you. Ask at the front desk or write to the grievance officer.
With the doctors treating you at this clinic, and with anyone you ask us to share it with. If you link an ABHA number, sharing through ABDM happens only with your consent, per request.
